Laboratory procedures worth more than P500.00 require approval from ValuCare office

File Philhealth during:
  • Out-patient OR procedures
  • Dialysis
  • Chemo/Radio Therapy
  • Cataract extraction
  • Endospic procedures
  • Others - as required
Submit duly accomplished Philhealth at the Billing Section prior to discharge & keep the acknowledgement receipt for future reference
